By signing below, I acknowledge that I am voluntarily participating in Embroidery Classes, Sewing classes, Crafting Classes, and Custom Apparel Decorating classes offered by VanDiesel Designs, LLC, located in Sicklerville, NJ. I understand that this activity involves the use of needles, embroidery machines, sewing machines, scissors, and other tools that may pose a risk of minor injury, including but not limited to needle pricks, cuts, or strain from repetitive motion.
By checking the box on the Electric Activity Release of Liability during the ticketing process, I acknowledge that I am voluntarily participating in Embroidery Classes, Sewing Classes, Crafting Classes, and Custom Apparel Decorating classes offered by VanDiesel Designs, LLC, located in Sicklerville, NJ. I understand that this activity involves the use of needles, embroidery machines, sewing machines, scissors, and other tools that may pose a risk of minor injury, including but not limited to needle pricks, cuts, or strain from repetitive motion.
I hereby acknowledge and agree to the following:
- Assumption of Risk I understand and accept that participation in embroidery activities carries inherent risks. I voluntarily assume full responsibility for any personal injury, accident, or loss that may result from my participation, including injuries such as needle punctures, cuts, or other minor accidents.
- Release of Liability I release and hold harmless VanDiesel Designs, LLC its owners, instructors, employees, volunteers, and affiliates from any and all liability, claims, demands, or causes of action arising out of or related to any injury, loss, or damage that may occur during or as a result of my participation in the embroidery classes.
- Medical Treatment I authorize VanDiesel Designs, LLC to provide or seek medical treatment in the event of an injury and agree to be responsible for any associated costs.
- Photography and Media Release I grant permission for VanDiesel Designs, LLC to use photographs or videos taken during the class for promotional purposes, unless I opt out in writing.
- Acknowledgment of Understanding I have read this release of liability and assumption of risk agreement, fully understand its terms, and sign it freely and voluntarily.
